How to Change Your Name on a New York Driver License

New York splits the license name change in two, and the split is the whole procedure. A standard license, permit, or non-driver ID can be changed by mail on form MV-44NC, if the Social Security number is on file with the DMV, the name has already been changed with the Social Security Administration, no commercial license is held, and no other information is changing. Everything else — Enhanced and REAL ID documents, commercial licenses, and any case with a second change — goes in person on form MV-44, with a new photo for REAL ID and Enhanced documents. The fee is the same either way: $12.50 for an amended permit or license, $5.00 for an amended non-driver ID card. The rejection rule is stated on the form itself: if you have not already changed your name with the Social Security Administration, your application will be rejected.

The mail route: standard documents only

Complete form MV-44NC, the Application for Name Change Only on a Standard Permit, Driver License, or Non-Driver ID Card. The form's conditions are cumulative: it is only for a standard document (all Enhanced, REAL ID, and commercial documents are excluded); no other information changes are allowed as part of the transaction; the name on the new Social Security card must exactly match the name requested; the Social Security number must be on file with the DMV; and copies of the current license or photo document and the new Social Security card must be provided. Provide a copy of one of the accepted proofs of the name change: a government-issued marriage certificate, a government-issued court order, an amended birth certificate, divorce papers that indicate a name change, or naturalization papers. The full signature, which will be used on the new document, must stay within the signature box or the application will be rejected. Enclose a check or money order for $12.50 (license or permit) or $5.00 (non-driver ID) payable to the Commissioner of Motor Vehicles, and mail to the Utica Processing Center, 207 Genesee Street, Utica, NY 13501.

The in-person route: REAL ID, Enhanced, commercial, and combined changes

When changing the name on a REAL ID or Enhanced license, permit, or non-driver ID, the photo must also be updated: bring a completed form MV-44 to a local DMV office, with the proofs the ID-44 guide lists for the document type. The same in-person route applies if the mail conditions are not met — a commercial driver license or learner permit, a second information change, or no Social Security number on file. The amended-document fee is the same $12.50 or $5.00. Bring the same proof set: the current document, the new Social Security card, and the government-issued proof of the name change.

The boundary of the transaction

The MV-44NC application affects only the photo document. A name change on the registration, title, or other DMV-issued documents requires a separate application for each, covered on the name-on-title page. And the order is not negotiable: Social Security first, DMV second, with the name matching exactly. A mismatch between the requested name and the Social Security card is a rejection, not a correction the DMV will make for you.
