Small Estate Affidavit Form Finder

Pick a state to find its small estate form, the office that issues it, and where to get it. Vendor-copy forms are flagged as vendor copies. Forms that were never opened are omitted with a note, never filled from a template site.

How to read the result

  1. Check the source tier. A form read from the issuing office outranks a vendor copy, a mirror copy, and a snippet read. The tier is stated in every entry.
  2. Form requirements are not statute requirements. A form can add a notary block, a death-certificate attachment, or a debts statement that the statute never states. The result keeps the two lanes apart.
  3. Stale forms stay flagged. Forms still printing a dead figure, such as Delaware county forms stating $30,000 and Vermont form 700-00001SM stating $10,000, are never presented as current.
  4. Open the state guide before acting. The state directory links every guide. A finder result is not a filing decision.

Source

Form names, numbers, issuing offices, and source tiers from research files A412 through A460 in the Veldica Traffic Ledgers backlog, with closure files A455 through A460 superseding base files where they correct a figure, plus the live directory exemplar for Colorado form JDF 999 and California forms DE-305 and DE-310. Open flags and omissions are disclosed in the result text. Facts verified 2026-10-31 through 2026-11-02.
