Wage Claim Form Finder
Pick a state to see which agency takes wage claims, which form or online system starts the claim, and the filing deadline the agency or statute states. Only states with a verified wage claim guide carry data here. A state with no verified guide says so plainly instead of showing another state's form.

How to read the result
- The result names the agency first. The agency is the office that takes the claim. A state with no agency route, such as Florida and Georgia, says so in the agency line instead of naming a form.
- A missing form name is an omission, not a gap to fill. Where no form name was verified, the result states the route that was verified, such as an online system, phone intake, or an unnamed department form. Never carry a form name from another state into a state with none stated.
- Deadlines run from different dates. Some run from the date wages were due, some from the date wages were earned, and some from the scheduled payday. The result states the trigger the source states. A state with no deadline figure verified says so.
- Open the state guide before filing. The result links the full state guide for the state. The guide carries the fields, the filing routes, the conflicts found, and the source list. A finder result is not a filing decision.
States covered
The finder carries verified data for 20 states: Arizona, California, Colorado, Florida, Georgia, Illinois, Massachusetts, Michigan, Minnesota, Nevada, New Jersey, New York, Ohio, Oregon, Pennsylvania, Tennessee, Texas, Virginia, Washington, and Wisconsin. Those are the 16 live state guides (Arizona, California, Colorado, Florida, Georgia, Illinois, Michigan, Minnesota, Nevada, New Jersey, New York, Ohio, Oregon, Pennsylvania, Texas, Washington) plus the 4 new state guides (Virginia, Tennessee, Massachusetts, Wisconsin), with the Colorado and Texas form guides deepening the Colorado and Texas cells rather than adding states. Every other state shows the not-verified result.
